check the following.

-connection ng HDD mo sa motherboard mo
-usb/cd-rom na installer mo ay bootable ba?
-fresh HDD partition(delete volume then create new partition - make sure to back up)

possible po na HDD yan. pwedeng may bad sector na sya.

if possible po pala change the title your thread para mas madami makatulong sa inyo.
