Sir help po,

pwde q na ba mgamet yang ra 9048? gusto q po baguhen name q kasi name ng lalake "jose" po eh dapat po josie po daw yun ngkulang ng "i" nsa tamang edad na din aq para ichange yung name q, Actually ginagamet q na ung name q na "jose" since h.s lng anu po ba 1st step na ggwen ? san po ba muna aq pupunta? pra maayus un at anu anu requirements.. ? please help me ..

sir ask ko lang po, kasi yung apelyido ko po sa BC ko SEBU ngayon po yung mga school records ko SEBO ang nkalagay... yung SEBU po ang gusto ko gamitin kasi yun po yung nakalagay sa BC ko paano ko po mapapaayos yung mga school records ko from SEBO gagawin SEBU...

I'm Back, medyo naging sobrang busy lang sa work ko pero i will answer all of your questions. isa isa lang po dapat kase may rules po ata tayo sa forum posting na 2 consecutive post lang.

... anyway about sa bc di kasi naka indicate dun sa bc ko kung san ako pinanganak ang nakalagay lang dun manila at yun name ng lying-in , nung kumuha ako ng passport sa dfa lucena nid daw na nakaindicate kung san sa manila at anung street ako pinanganak ngaun nilagay sa application form ng dfa ko yun supplementary report? panu po ba yun? at magkanu magagastos ko dun?

and panu po kung may mali isang letter sa ipilyido panu po yun?

Hi! Sir,

Punta ka sa City hall kung saan ka pinanganak then dun ka po mag pa supplementary.. mga 200 pesos pataas kang po yan sir kaya wag ka pong magpa loko..kase ang pinaka worst ay ang city hall ng manila. andun lahat ng kalokohan. yoko na mag drop ng mga kalokohan nla baka mapahamak pako maximum of 3 months po ang process nyan..


then yung maling letter naman po punta ka po sa munisipyo (Cityhall) kung saan ka pinanganak local civil registry..correction of entry of R.A. 9048 po ang tawag dyan. dun mo po ipa correct yung name mo.. nasa 1,500 to 2500 po yan at 3 months maximum po ang pag aantay sa pag ayos nyan..

Court hearing (korte) po pag numbers ang involve. at sa city hall nyo po papa korte yun..malaki po ang gagastusin nyo dun.

Correction of entry (RA 9048).. Punta po kayo sa munisipyo kung saan po kayo ipininganak at mag dala rin po kayo ng authenticated copy ng birth certificate nyo galing po ng NSO. duon nyo po itanong kung magkano ang pa correct nyan.

mg ask lng sna aq anu b tlga ung cnsbing "Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO depending on the NSO signatory."
I am planing to apply for my son's passport who is 2mos old. panu po ako mkakakuha nito at gnu po kya ito ktgal?

Ask kp lng rin po about sa birth certificate regarding dun sa Birth date ksi ug original bday ko is Dec,12 1988 But ung nasa NSO ko is May 12,1995 paano ko po b un mapapacorrect....na walang mababagao sa lahat ng requirments ko......help me pleased.

i tried to get his BC from NSO pro sb nila ala p cla ng BC kya i need to get a copy from d city hall.. den ibblik s knla dw... nguluhn aq kc i thought dat aftr 2mos. mron n cla.. i hop lng it wont take long to process.. tnx for the reply sir.. more power to this thread po...

Court hearing (Korte) po yung case nyo but in your case hindi na po papayag na ipa bago pa kase malayo na po ang month at ang year ng birth nyo po. Pero try nyo parin po. Malaki po gagastusin nyo dyan.

having problem sa nso birthcert ko,hindi daw ihohonor ng dfa.wala daw kasing stamp,dko alam kung saan ko papa stamp e.tulong naman jan. yung stamp is coming from the office of the civil registrar.
